General Summary:
Utilizes the nursing process as a framework for practice in the care of clients. Provides professional client care utilizing the standards of psychiatric and medical nursing care in accordance with the Agency’s policies and procedures and under the supervision of the Residential Manager. Demonstrates individual professional accountability and responsibility. Program management and the Agency’s mission, values and vision are supported.
Essential Job Functions:
· Assessment:
1. Assesses clients through the collection of the following data: biophysical, psychosocial, environmental, self care, educational and discharge factors.
2. In concert with the client, collects data needed to assess the client’s nursing care needs. Other family members, significant others, staff, and health care providers will contribute to the data collection process as appropriate.
· Diagnosis:
1. Reviews collected data to facilitate formulation of the nursing diagnosis. Utilizes the nursing diagnosis to determine the nursing plan of care and prioritizing the client’s needs.
· Planning:
1. Applies appropriate theory that is scientifically sound as the basis for the development of the nursing plan of care with specific goals and interventions that direct actions to address each client’s need.
2. In collaboration with the client identifies goals that are realistic, measurable and achievable.
· Intervention:
1. Utilizes concepts and principles from nursing theory and from the biological and behavioral sciences to implement planned nursing actions that promote, maintain or restore optimal physical and mental health, prevent illness, and contribute to the integration of functioning.
2. Initiates action in response to information indicating risk(s) to correct, prevent and/or minimize risk(s) to the client, staff, and/or Agency.
3. Communicates with the physician and/or physician extender, providing status reports, review of the client progress as well as accepting orders in accordance with Agency policies and procedures.
4. Administers medications and/or supervises client self-administration of prescribed medications.
· Evaluation:
1. Continuously analyzes the client’s response to nursing actions in order to evaluate the degree of goal attainment and to revise the nursing diagnosis and the nursing plan of care.
2. In collaboration with the client actively:
a. Evaluate the client’s progress toward the established goal
b. Identify elements that promote or deter progress
c. Plan changes in interventions or revise the interventions that enhance goal achievement
· Professional Performance:
1. Participates in program evaluation and continuous quality improvement activities to enhance the care and treatment of the clients at Catholic Charities.
2. Accountable for their continued professional growth. Opportunities are provided for inservice education. Also contributes to the professional growth of other staff within the Agency.
3. Collaborates with the other members of the multidisciplinary team to assess, plan, implement, and evaluate client care.
4. Uses communication skills with individuals that (are intended to) achieve the desired outcome, be perceived as satisfying and acceptable by those involved and/or promote teamwork.
5. Provides professional role modeling to staff and clients.
6. Performs other duties as assigned.

Knowledge, Skills, and Abilities:
· Knowledge of and commitment to Catholic Charities Mission, Values, and Vision.

Education and Experience:
· Licensed as a Licensed Practical Nurse within the state of
· Minimum of 1 year nursing experience with experience in addiction/detoxification preferred.
