Position Description

Job Purpose:
To provide family support/community treatment aide services to individuals and families involved in mental health therapy.

Key Responsibilities: 
Direct Service Delivery Hour Requirements:
• FT salary requires 28 direct service hours per week for 44 weeks, which equals 1232 hours per year.

Direct Service Delivery:
• Provide direct services to individuals and families referred to the Family Support/CTA Program by various sources.
• Work under the guidance of HHSs case managers, therapists, and families to implement the Agency Home-Based Services Program, and assistance in the development of the Treatment/Service Plan.
• Teach skills necessary to assist in meeting the mental health treatment plan goals to enable children to remain in the home or be returned home safely after our of home placement.
• Provide services to the families in their own home and at times that are convenient for them.
• Provide intensive, crisis resolution and follow-up in consultation with the therapist.
• Maintain ongoing verbal and written contact with HHS case managers and/or referring therapist/provider system.
• Have and develop knowledge and understanding of the referring therapist/program, and for service delivery and reimbursement.
• Participate in team meetings and referral representatives as needed or requested.
• Provide accurate timely written reports to case managers, supervisors and other agencies as requested in addition to completing routine monthly reports.
• Services for child may include skill building, transportation of the child (pre-authorized), communication with the school staff and other interested significant others as outlined in the plan.

Record Keeping:
• Maintain ongoing records necessary for licensure, accreditation and third party payments.
• Complete monthly reports as required by HHS and the referring therapist/program.
• Complete all necessary service and statistical records.

Growth and Development:
• Attend and participate in staff meetings as requested for the purpose of program planning and assessment, and personal and professional growth and development.
• Through individual reading, workshops, videotape presentations and case consultations, work to build support work skills to meet referral source expectations and provide the highest quality services.
• Attend required pre- and in-service training.

Job Requirements:

• Bachelors degree in Social work, Psychology or other mental health related field; or be 19 years old with a high school diploma and have at least 3 years experience working with families and children.
• Personal convictions that are compatible with the philosophy and goals of the Agency as well as demonstrate a commitment to empowering families to solve their own problems and to the value of a nurturing family as the ideal environment for a child.
• Valid driver’s license, proof of liability insurance and ability to drive for Agency business.
• Commitment to the mission of Lutheran Family Services.
